About Benchmark

Four providers. One team that already knows you.

Care needs change. A patient who comes home after surgery may later need more support than home visits can provide, or a family may need comfort care at the end of life. Because each stage is handled by a Benchmark company, you never have to start over with strangers or chase a new referral.

Every Benchmark company is licensed in California, staffed by our own clinicians, and coordinated by the same care team. We are non-denominational and faith-based, and we have served Southern California families since 1997.

No referral needed to start home health. Medicare requires a physician’s order, and our own physician writes it and completes the face-to-face visit, so you do not have to chase one down.
Hands-on care by our own team. Nurses and therapists employed by Benchmark, not a registry.
Direct hospital and physician-network contracts for faster authorizations from hospital to home or facility.

Skilled nursing and therapy at home

Intermittent visits from nurses, physical, occupational and speech therapists, medical social workers, and home health aides, following a physician’s plan of care. For recovery after surgery or hospitalization, wound care, medication management, infusion therapy, and chronic conditions.

Who it is for: patients who are homebound and need skilled care a few times a week.
Who pays: Medicare-certified; see our Insurance page for the plans we work with.
Not included: hourly or live-in caregivers. Home health is visits, not around-the-clock help.

24-hour skilled nursing for catastrophic injury and complex illness

Two Congregate Living Health Facilities (CLHFs) in the San Fernando Valley: licensed health facilities with 24-hour skilled nursing in a home-like residence. Care is more intensive than a traditional nursing facility, for brain and spinal cord injury, ventilator dependency and weaning, tracheostomy, complex wounds, neuromuscular disorders, and kidney care with outpatient dialysis.

Who it is for: medically complex adults, often referred by hospital case managers and ICU or trauma discharge planners.
Who pays: Medi-Cal, the HCBA waiver, and health-plan, hospital and IPA contracts.
Saint Mariam Congregate keeps a 1:3 staff-to-resident ratio; Best Quality Living is licensed by the California Department of Public Health.

Saint Mariam Hospice Services

Hospice: comfort, dignity and support for the whole family

Pain and symptom management, palliative nursing, caregiver and spiritual support, and bereavement care, wherever the patient calls home. Joint Commission accredited.

Who it is for: patients with a life-limiting illness who are choosing comfort over curative treatment.
Support that does not clock out: a hospice nurse is on call by phone 24 hours a day for our patients.

Complex case placement for hospitals and case managers

When a patient’s needs do not fit a standard discharge, our coordinators assess the case, confirm coverage, and place the patient at the right Benchmark provider or an appropriate outside facility. Admissions and placement support are available 24/7.

Who it is for: discharge planners, ICU and trauma teams, workers’ compensation and liability case managers.

How it works

What happens after you call

Tell us what is happening

A care coordinator listens, asks about the diagnosis, the hospital or doctor involved, and where the patient is now. No forms to fill out first.

We verify coverage and arrange the order

We check eligibility with the patient’s plan and, for home health, our physician writes the order and completes the required face-to-face visit.

Care starts

A nurse completes the first assessment at home, or admissions coordinates the move into one of our facilities, and the same team stays with you if needs change.

Direct contracts mean faster authorizations and a smoother handoff from hospital to home or facility. We also work with multiple IPA and MSO physician networks across the five counties.
